download, print, and complete the following Patient Information
Sheet prior to your child's first appointment. If you are
unable to print this form, please plan to arrive at least 15 minutes
prior to your child's scheduled appointment.
Dr. Arnold Kagan is a private pediatrician serving the
Pearland, Houston, and surrounding areas to provide excellent health
care and preventative services to children.

immunization record with you.
